CHIEF PETTY OFFICER DOWNCREOUX S SUPERLATIVE EFFORTS IN SUPPORT OF THE AIR OPERATIONS MISSION HAVE BEEN TRULY OUTSTANDING. For professional achievement in the superior performance of her duties while assigned to Naval Branch Health Clinic, Mayport, FL from 31 DECEMBER 2008 TO JULY 2010. Petty Officer Owens has performed as a Psych Tech in the Mental Health Clinic. Manage a mental health clinic that is very high demanding. She provides all medical support for two providers; whom provide care for more than 720 patients. Maintains providers templates, schedules, appointments, referrals, and runs the monthly statistic reports in CHCS and AHLTA. Triage over 300 emergency patients from the ships and base. Responsible for conducting Minnesota Multi-Phasic Personality Iventory (MMPIs); personality tests on patients requiring testing for personality disorders, and Millon Clinical Multiaxial Inventory (MCMI); is a psychological assessment on patients during initial screening. Petty Officer Owens professionalism and devotion to service reflect great credit upon herself and were in keeping with the highest traditions of the United States Naval Service.

DEMONSTRATING PROFESSIONAL KNOWLEDGE AND SOUND JUDGEMENT AS THE DIRECTORATE SUPPLY PETTY OFFICER, PETTY OFFICER HAWKINS DIRECTLY CONTRIBUTED TO MISSION ACCOMPLISHMENT AND STAFF MORALE. For professional achievement in the superior performance of his duties while assigned to Pearl Harbor Naval Ship Yard. Petty Officer Raster has distinguished himself as a key sailor at Pearl Harbor Naval Shipyard making special mention twice in the NAVSEA Newsletter. During his Annual Training March 2009 and June 2009 he stood out in the eyes of his supervisors and shipyard workers . He assisted in induction, screening, troubleshooting and repairing circuit boards and amplifier assemblies saving the Navy $120,000 repairing circuit boards that would have been otherwise scrapped. He exemplifies the mission of Surge Maintenance for critical missions of Naval ship yards. Later in his tour his demonstration of superior trade knowledge allowed him to shine once again in which he rapidly integrated with civilian shipyard skilled trades and quickly picked up the processes for refurbishment of USS REUBEN JAMES (FFG-57) M61A1 Vulcan Cannon and the ammunition handling system of the MK15 Phalanx CIWS. Petty Officer Raster performed his duties with no supervision and a positive attitude. His contribution was instrumental to the direct support and mission accomplishment of Peral Harbor Naval Shipyard and Intermediate Maintenance Facility which saved 336 manhours. His initiative and tenacity in completing NAVSEA operational requirements and volunteering for new programs to assist Fulltime Support Personel at the Naval Operational Support Center Minneapolis are in keeping with the highest examples of the Navy's CAN DO spirit. His outstanding accomplishments during this tour merit the special recognition afforded by the awarding of the Navy and Marine Corps Achievement Medal. Professional achievement as administrative Petty Officer, Branch Dental Clinic, Naval Air Station, North Island, Directorate for Dental Services, Naval Medical Center, San Diego, California from July 2008 to July 2009. Petty Officer Juarez assumed the administrative role for the clinic during increased operational tempo. She managed the daily operations, appointment schedule and work assignments of 36 staff providing support to 25 dental providers. Additionally, she headed the military medical support program and insured that 427 patients were seen for out source dental treatment. Her efforts contributed to achievement of an operational dental readiness of 96 percent and a dental health index of 65 percent for 7,900 beneficiaries assigned to 48 tenant commands and 21 squadrons. Petty officer Juarez s professionalism, personal initiative and unswerving devotion to duty reflected credit upon herself and were in keeping with the highest traditions of the United States Naval Service.


PROFESSIONAL ACHIEVEMENT AS NETWORK ADMINISTRATOR DURING AN EXTENDED DOCKING PLANNED MAINTENANCE AVAILABILITY PERIOD, USS GERMANTOWN (LSD 42), FROM NOVEMBER 2008 TO OCTOBER 2009. PETTY OFFICER HERNANDEZ CONSISTENLY PERFORMED HIS DEMANDING DUTIES IN AN EXEMPLARY AND HIGHLY PROFESSIONAL MANNER. HIS MANAGEMENT SKILLS AND NETWORKING KNOWLEDGE PROVIDES THE FOUNDATION TO GERMANTOWN'S ABILITY TO EXCEL ON ALL LEVELS OF COMMUNICATIONS. CORRECTED OVER 500 COMPUTER RELATED TROUBLE CALLS. PETTY OFFICER MCNEIL CONSISTENTLY PERFORMED HIS DEMANDING DUTIES IN AN EXEMPLARY AND HIGHLY PROFESSIONAL MANNER. AS A SHIP S STORE OPERATOR HE KEPT A NEAR FLAWLESS INVENTORY AND RAISED OVER $30,000 IN REVENUE FOR WASP S MORALE, WELFARE AND RECREATION PROGRAM. ADDITIONALLY, PETTY OFFICER MCNEIL GAVE OVER 500 MILITARY REGULATION HAIRCUTS TO MEMBERS OF WASP S CREW WHILE SERVING AS A SHIP S BARBER. HIS PERSONAL INITIATIVE AND ATTENTION TO DETAIL HAVE BEEN INSTRUMENTAL IN THE DIVISION S SUCCESS SINCE HIS ARRIVAL. FOR OUTSTANDING ACHIEVEMENT WHILE SERVICE AS DECK DIVISION CHIEF PETTY OFFICER HECTOR JAMES FEARLESS ABOARD USS MCGRAW (DDG 888) FROM MARCH 2003 TO NOVEMBER 2006. CHIEF PETTY OFFICER FEARLESS CONSISTENTLY PERFORMED HIS DEMANDING DUTIES IN AN EXEMPLARY AND HIGHLY PROFESSIONAL MANNER. BECAUSE OF HIS GREAT DEVOTION TO DUTY AND ATTENTION TO DETAIL HE SUPERVISED OVER 200 DECK EVOLUTIONS DURING HIS TWO DEPLOYMENTS INCLUDING 35 UNREPS, 14 ANCHORINGS, 2 MOORINGS TO A BUOY, 2 TOW-AND-BE-TOWED EXERCISES, AND HUNDREDS OF HELO AND SMALL BOAT OPERATIONS, INCLUDING VBSS AND THE IRAQI ARMS, ALL FINISHED WITH NO SAFETY ISSUES. HIS EXCEPTIONAL EFFORT TO GO ABOVE AND BEYOND THE CALL OF DUTY LED HIM TO BE INVOLVED HIM IN SUCH ROLES AS NAVY RELIEF COORDINATOR FOR THE SHIP, AND EARNING A BLACK BELT IN MARTIAL ARTS IN HIS SPARE TIME. BY HIS UNSWERVING DETERMINATION, WISE JUDGMENT, AND COMPLETE DEDICATION TO DUTY, CHIEF PETTY OFFICER FEARLESS REFLECTED CREDIT UPON HIMSELF AND UPHELD THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.



PROFESSIONAL ACHIEVEMENT IN THE SUPERIOR PERFORMANCE OF DUTIES WHILE SERVING AS CLASSIFIED MATERIALS CONTROL CENTER OFFICER, HEADQUARTERS BATTERY, 3D LOW ALTITUDE AIR DEFENSE BATTALLION, MARINE AIR CONTROL GROUP, 3D MARINE AIRCRAFT WING, MARINE FORCES PACIFIC FROM 2 OCTOBER 2003 TO 25 DECEMBER 2003. DURING THIS PERIOD, MASTER SERGEANT ROCK PERFORMED AS A CLASSIFIED MATERIALS CONTROL CENTER OFFICER IN A MANNER THAT DRAMATICALLY INCREASED EFFICIENCY. THROUGH A COMPLETE RESTRUCTURING OF THE CLASSIFIED MATERIALS CONTROL CENTER, HE RAISED THE CENTER'S RATING FROM UNSATISFACTORY TO MISSION CAPABLE IN LESS THAN 30 DAYS.   MASTER SERGEANT ROCK'S DISTINCTIVE ACCOMPLISHMENTS, PROFESSIONALISM, AND STEADFAST DEVOTION TO DUTY REFLECTED GREAT CREDIT UPON HIM AND WERE IN KEEPING WITH THE HIGHEST TRADITIONS OF THE MARINE CORPS AND THE UNITED STATES NAVAL SERVICE.



PROFESSIONAL ACHIEVEMENT IN THE SUPERIOR PERFORMANCE OF HIS DUTIES WHILE SERVING AS INFORMATION SYSTEMS MANAGER, JOINT FORCES COMMAND, NORFOLK, VIRGINIA FROM 12 APRIL 2006 TO 15 JANUARY 2007.   DURING THIS PERIOD, MASTER SERGEANT MAELSTROM, WHILE MAINTAINING HIS REGULAR DUTIES, LED A TEAM IN THE ENGINEERING OF PHYSICAL AND INFORMATION ASSURANCE SECURITY CONTROLS NECCESSARY FOR THE RELOCATION OF THE DISA HEADQUARTERS.   BY UTILIZING HIS EXPERTISE AND LEADERSHIP IN THE DESIGN AND PROTECTION OF THE NEW DISA FACILITY, MORE THAN TEN THOUSAND DOLLARS IN OUTSOURCING COSTS WAS SAVED.  MASTER SERGEANT MAELSTROM'S INITIATIVE, PERSEVERANCE, AND TOTAL DEDICATION TO DUTY REFLECT CREDIT UPON HIMSELF AND ARE IN KEEPING WITH THE HIGHEST TRADITIONS OF THE MARINE CORPS AND THE UNITED NAVAL SERVICE.

SEAMAN MUNCHEN DISTINGUISHED HIMSELF BY REFERRING SEVEN INDIVIDUALS WHO ENLISTED AS MEMBERS OF THE NAVAL RESERVE. THESE ENLISTMENTS WERE AN IMPORTANT CONTRIBUTION TO THE OVERALL READINESS OF THE NAVAL RESERVE AND THE DEFENSE OF THE NATION. For superior achievement in the performance of her duties while serving as Reactor Controls Leading Petty Officer for the nuclear propulsion section, Engineering Division, aboard the USS BAINBRIDGE (DDG 96) from 15 Februray to 15 June 1999, while deployed with the Fifth Fleet to the Persian Gulf, North Arabian Sea, and the Strait of Hormuz. Petty Officer Johnson's extraordinary performance had a significant impact on the successful participation in operation SOUTHERN WATCH as well as the effective execution of Maritime Intercept Operations in the Arabian Gulf under the control of the Commander, U.S. Naval Forces, Central Command. Displaying notable initiative, leadership, and dedication to duty as a nuclear propulsion specialist, Petty Officer Johnson authoritatively guided her workcenter through a challenging and prolonged deployment and a successfull trans-Atlantic crossing. Petty Officer Johnson's exemplary performance was recognized and praised by the Engineering Division, which found no significant discrepancies with any program under her watch. Petty Officer Johnson's professionalism, initiative and enduring devotion to duty reflected great credit upon herself and her ship and is in keeping with the highest traditions of the United States Naval Service.



PROFESSIONAL ACHIEVEMENT IN THE SUPERIOR PERFORMANCE OF HER DUTIES AS BUILDER FOR NAVAL MOBILE CONSTRUCTION BATTALION TWENTY-ONE FROM 12 APRIL 2004 TO 27 MAY 2004. DISPLAYING EXCEPTIONAL SKILL, RESOURCEFULNESS AND INITIATIVE, SHE ORGANIZED AND SUPERVISED THREE PROJECTS FOR NAVAL RESERVE CENTER (NRC), HAWAII FROM INCEPTION TO COMPLETION. HER EXEMPLARY EFFORTS IN THE MANAGEMENT OF CREW WORK FORCE, OBTAINING MATERIALS, AND SCHEDULING OF WORK WENT BEYOND THE CALL OF A JUNIOR PETTY OFFICER. HER INNOVATIVE IDEAS AND EFFORTS HAVE RESULTED IN THE SAVING OF OVER $20,000 IN LABOR AND MATERIALS. PETTY OFFICER HENRIETTA JONES' PROFESSIONALISM AND DEVOTION TO DUTY REFLECTED CREDIT UPON HERSELF AND WERE IN KEEPING WITH THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.
