In this capacity, he provided medical oversight to a complex, multi-unit exercise environment, ensuring the seamless execution of 639 medical evaluations and procedures, as well as the completion of 81 explosive ammunition handler and driver licensing medical assessments, directly supporting combat readiness and ensuring safe handling of high-risk occupational duties. His involvement ensured that medical risk was expertly managed while enabling training to continue uninterrupted and at full operational tempo.

During ITX 3-24, Commander Santiago supervised a medical team of 8 Officers and 69 Hospital Corpsmen, directing daily operations, coordinating patient flow, and ensuring all clinical and operational functions were executed to the highest standard. He maintained full accountability of more than $1.2 million in medical supplies, pharmaceuticals, and equipment, ensuring that vital medical resources were positioned and employed effectively throughout distributed training sites. His command presence, disciplined oversight, and deep understanding of operational medicine ensured that the Regiment's medical capability remained fully responsive and mission-focused at all times.

Beyond his field performance, Commander Santiago made significant and lasting contributions to improving the medical readiness and administrative performance of the Regimental Headquarters Company. Through his implementation of streamlined administrative processes, improved medical tracking systems, and targeted readiness interventions, Headquarters Company saw a greater than 6% increase in medical readiness during his first year. This improvement reflected his meticulous attention to detail and his ability to identify systemic gaps and design sustainable, efficient solutions that strengthened the Regiment's operational posture.

Commander Santiago's operational skill and clinical judgment were repeatedly demonstrated through decisive action during medical emergencies in austere environments. While serving as the senior medical provider during field operations, he personally responded to a Marine who had fallen and suffered a significant shoulder dislocation. Recognizing both the severity of the injury and the difficulty the onsite team faced in reducing the shoulder, he assumed direct control of patient care. Working within the limitations of a field tent and without access to sedation or advanced equipment, he successfully performed a closed reduction of the dislocation. His expertise allowed the Marine to return to duty within 48 hours, prevented costly evacuation or lost training days, and exemplified the highest standards of operational medical practice.

Similarly, during a return convoy from the field, Commander Santiago was alerted to a junior Marine experiencing acute respiratory distress. With no medical equipment available and operating inside a moving vehicle, he conducted a complete assessment, identifying severe wheezing and signs consistent with an asthma-like exacerbation. Recognizing the life-threatening nature of the condition, he coordinated with the Marine's chain of command and the exercise officer-in-charge to divert the convoy to the nearest emergency department along the route. Commander Santiago remained with the Marine for over an hour, providing continuous reassurance, coaching breathing techniques, and monitoring for deterioration. Upon arrival at the hospital, he performed a precise turnover to emergency staff, enabling immediate treatment. Due to his actions, the Marine was stabilized, treated, and returned to full duty within three hours. His rapid assessment, calm decisiveness, and expert field medicine directly averted a potential tragedy and demonstrated his profound commitment to the welfare of the Marines under his charge.

In addition to his operational responsibilities, Commander Santiago was deeply invested in strengthening medical knowledge and clinical competency across Navy Medicine. He provided 40 hours of Advanced Trauma Life Support (ATLS) instruction, directly resulting in the certification of 42 Defense Health Agency providers and enabling two instructor candidates to complete their instructor qualifications. His contributions significantly enhanced the Navy and Marine Corps' trauma readiness pipeline, directly supporting the Navy Surgeon General's Blue Horizon initiative and improving medical capability across multiple commands.

Commander Santiago also distinguished himself through his exceptional mentorship and leadership development efforts. He provided direct career guidance, performance counseling, and professional mentorship to nine junior officers, resulting in two officer promotions and supporting the selection of one officer as the United States Naval Reserve Junior Officer of the Year. Under his direction, every deployer received timely, comprehensive medical screening and care, ensuring full readiness for the rigorous operational environment they were about to enter.

During Exercise SEMPER DURUS, he oversaw medical operations for 454 Marines and Sailors, managing medical coverage for 10 ranges and training events, ensuring seamless and safe execution of all training objectives. His team successfully responded to multiple emergency medical situations—including one requiring advanced field care and safe transfer to a higher level of care—each handled under his direct leadership and oversight. Additionally, he coordinated the delivery of Tactical Combat Casualty Care (TCCC) instruction to 20 Marines, materially improving lifesaving capabilities across the force.

Beyond his assigned duties, Commander Santiago provided extraordinary service to Navy Medicine and the broader operational medical community. As the Assistant Specialty Leader for Operational Medicine in the Navy Reserve, he played a crucial role in recruiting medical providers to operational billets supporting Marine Forces and the Naval Construction Force. His expertise was integral in updating the Navy Reserve Master Practice List for General Medical Officers, strengthening readiness and shaping future policy affecting the Medical Corps' operational capability.

Always willing to accept challenging missions, Commander Santiago volunteered to serve as the Acting Third Fleet Surgeon for Exercise KEEN EDGE 26, a bilateral command post exercise between U.S. Forces Japan and the Japan Joint Staff. His participation enhanced regional combat readiness, strengthened interoperability between allied medical forces, and demonstrated his ability to perform at the highest levels of operational responsibility.
