Meritorious service while serving as Medical Leading Petty Officer, Navy Region Mid-Atlantic Reserve Component Command, MEDHOLD, from October 2019 to June 2022. During this period, HM2 Bourgeois assumed the mantle of military medical case manager and TRICARE registrar in response to the challenges of the COVID-19 pandemic and subsequent SURGMAIN mobilizations. His tenacious leadership and continuous pursuit of process improvement led to a full restructuring of MEDHOLD patient intake and tracking processes, 249 patients returned to Fit for Full Duty status, 19 patients transferred to the Line Of Duty program, and 97 patients medically separated or retired. HM2 Bourgeois' distinctive accomplishments, unrelenting perseverance and steadfast devotion to duty reflected credit upon himself and were in keeping with the highest traditions of the United States Naval service.
LIEUTENANT TIMOTHY DEMONSTRATED OUTSTANDING NURSING AND MANAGERIAL SKILLS AS TEAM LEAD NURSE AND SCHEDULING OFFICER IN FAMILY MEDICINE FOR FOUR MEDICAL HOME PORT TEAMS ENCOMPASSING 61 PROVIDERS AND 70 SUPPORT STAFF TO ENSURE 250 DAILY APPOINTMENTS FOR OVER 15,000 BENEFICIARIES. SHE IMPLEMENTED AN INTEGRATED HEALTH COMMUNITY INITIATIVE WITH TWO CASE MANAGERS AND FOUR CARE NAVIGATORS RESULTING IN FISCAL YEAR 2013 PROJECTED OUTCOMES COST SAVING OF 3,163,000 DOLLARS, 322 FEWER EMERGENCY ROOM VISITS, 71 FEWER ADMISSIONS, AND 3,661 FEWER SPECIALTY VISITS.
For outstanding performance and professional achievement of his duties while assigned to Naval Branch Health Clinic Belle Chasse, Louisiana from September 2020 to November 2021. Hospital Corpsman Morris, a general duty corpsman, served as the subject matter expert in the Behavioral Health Department. He managed a mental health clinic and provided medical support for seven providers, four of them through telemedicine, and collectively provided care for more than 1200 patients. He maintained providers' templates, schedules, appointments, referrals and served as the Departmental Case Manager. As the Behavioral Health Case Manager, he ensured the coordination and continuation of care for patients requiring a higher echelon of care. By ensuring inpatient psychiatric patients had a follow-up within two to five days of release, he maintained a HEDIS measurement in the 90th benchmark. He has disposition and assigned over 350 referrals from primary care managers to ensure patients receive adequate mental health care within 30 days of requesting care. As the telehealth coordinator, he was responsible for the implementation of the telemedicine program, providing much needed psychiatric care to over a hundred patients, saving the Navy an estimated $80,000 in a six month time frame. Hospital Corpsman Morris' initiative, professionalism and total devotion to service reflected great credit upon himself and were in keeping with the highest traditions of the United States Naval Service.
Captain Jonathan K. Smith distinguished himself by meritorious service while serving as Case Manager, 383rd Training Squadron, 373rd Training Group, Fort Sam Houston, San Antonio, Texas. During this period, Captain Smith was instrumental in the leadership of PACAF's largest outpatient military training facility supporting Air Force's largest overseas combat Wing. Captain Smith expertly assessed 200 medical charts managing 70 cases, decreasing appointment utilization by 50 percent, allowing management of care at 98 percent, earning best in PACAF. He authored a three hundred thousand dollar civilan provider contract which fortified 30 percent staffing, sustaining the ready-to-fly rate of 95 percent. His astute skills and determination led to his completion of four transoceanic critical missions, accumulating 42 flight hours, and saving four lives. During his missions, Captain Smith meticulously trained joint Department of Defense members and international members on the Joint Medical Attendant Transport Team and Disaster Medical Assistance Teams, capturing 20 outstanding awards and earning the PACAF Ambulatory Nurse for 2010. The distinctive accomplishments of Captain Smith reflect credit upon himself, the 383rd Training Squadron and the United States Air Force.
