AS PHA COORDINATOR HE MANAGED PHYSICAL HEALTH ASSESSMENTS FOR 14,000 PERSONNEL IN 44 TENANT COMMANDS. PETTY OFFICER MARTIN FILLED A CRITICAL GAP AND IMPLEMENTED THE NEWLY ENACTED PROGRAM, COORDINATING CARE FOR OVER 80 OF THE NETWORK'S MOST CRITICALLY ILL AND HIGHEST UTILIZING PATIENTS. SERVING AS A CONDUIT OF INFORMATION BETWEEN PATIENTS AND ALL MEMBERS OF THEIR HEALTHCARE TEAM, HE COORDINATED INTERDISCIPLINARY TEAM MEETINGS WITH TOP LEADERS OF HEALTH CARE RESULTING IN 100 PERCENT PROCESS IMPROVEMENT AND STREAMLINED PATIENT CARE.
