Family Practice Clinic Evals



DEDICATED LEADER. Led and mentored 54 Sailors, 47 junior officers, 24 civilians ISO 48 providers in the delivery of care through 28K encounters with a 94% patient satisfaction rate. Regular interaction resulted in 52 evaluations authored, 27 advancements, 17 award approvals, 7 college enrollments, 5 USMAP and 2 undergraduate completions.
-STRONG SAILORIZATION. Completed 80 CDBs, resulting in 14 workshop, 13 C-School selections, 11 extensions, 7 college enrollments, 5 USMAP certifications and 2 reenlistments with SRBs totaling $160K. Efforts were instrumental in achieving a 98% Career Information Program Review score.




VERSATILE CORSPMAN. Assisted a team of five Medical Providers and three Independent Duty Corpsmen in delivering care to 1,217 patients. Conducted comprehensive history assessments, vital signs checks, physical evaluations. Conducted nine in-grown toenail removals, seven suture removals, and 39 EKGs with zero mishaps.




As Team Lead of the Family Practice clinic, provided daily assistance to 4 providers, 4 nurses, and 7 Corpsmen in the delivery of patient centric care to empanelment of 3,790 active duty, dependents, retired, and other beneficiaries resulting in 19,844 encounters. Was directly involved in the completion of 2,500 patient encounters, 841 lab tests, 329 procedures, and the prescription of more than 2,000 medications. Spearheaded the task of a multi clinic late stay schedule between Family Practice and Internal Medicine.
Transmitted EKGs from Family Practice and Internal Medicine to the cardiology department, for fast and safe transmission of EKG results. As a representative for both safety and infection control, made sure clinical operations went smooth and concise while also ensuring patient health and safety was never in jeopardy.
