Physical Therapy Evals


Managed the scheduling of 1800 patient encounters for five providers in the rehabilitative treatment of 8500 beneficiaries in support of four FDNF-E ships with 100 percent satisfaction rate. As Physical therapy and occupation therapy clinic manager assigned 746 patient referrals to assure maximum access for rehabilitative services for pre and post op patients.




KNOWLEDGABLE IN RATE PERFORMACE. Solely provided medical care for 10 postsurgical inpatients; resulted in full recovery and minimal loss of manpower. Triaged and treated 12,225 sick call patients to keep a deployed crew fully medically ready.

IMPECCABLE WORK ETHIC. Took immediate action upon arrival and began working without therapists supervision in the Physical Therapy Department. His knowledge and experience has allowed for a 150% increase in available slots for patient appointments.

-TRUE TEAM PLAYER. Led five junior Sailors in administering 247 immunizations and verifying 483 health records for shipwide Periodic Health Assessment evolution.

-ENERGETIC SELF STARTER. Aggressively completed Adv DC 310 qual and volunteered to become lead Corpsman for FLYING SQUAD. Participated in 32 at sea fire party drills and four actual casualties, resulting in zero personnel casualties and minimal damage to the material condition of the ship.




As Leading Petty Officer of Physical Therapy, HM2 Lancer expertly led 10 sailors and civilian employees in providing quality patient care for thousands of active duty service members, enhancing force health protection and operational readiness. As command FEP Coordinator he implemented the new physical readiness test standards across the command. Conducted 25 fitness enhancement program workouts. Facilitated watch-standing training to over 50 sailors, contributing in the qualification of new POOWs and OODs increasing readiness, and improving situational awareness.




With great initiative, Lt Maximo championed the USS Frank Cable's (AS 40) first Physical Therapy Program at sea, rehabilitating numerous (11) patients, maximizing operational capacity during a recent underway. Furthermore, he facilitated a collaberative program with Lt Chavez at Naval Hospital Guam in the Physical Therapy Department, establishing a legacy of mutual cooperation and physical rehabilitation success for the greater good of the command.




Physical therapy LPO trained Two Junior Techs and provided OJT training to six Quad Zero Corpsmen, allowing for completion of 3229 Patient encounters at a network deferment value of $1,240,910. Applied Lean Six Sigma standards to clinic operations, reducing the Patient cancelation and No-Show rates to 5.0%, a 70% reduction from the previous year. Implemented space utilization practices capturing 10% more patient treatment area and allowing expansion of services to include functional rehabilitation program, resulting in a 20% increase in appointment availability.




While assigned to NATO Role 3 Multinational Medical Unit, HM3 Hartley served in the Physical Therapy Department in both the Role 3 and Warrior Recovery Center (WRC). HM3 managed 4 provider schedules for a total of 750 montlhy vistis resulting in a 95% return to duty rate. He also provided care to over 300 battle wounded patients at both the WRC and on the Intermediate care ward at the NATO Role 3. HM3 expertly served as both supply petty officer and assitiant equipment manager, maintaing $50 thousand in supplies and $250 thousand in equipment.
